In order for the vetting process to proceed, you must complete this application in its entirety and submit all the required information and records. Incomplete applications will not be processed and they will be returned to you. If you feel a question or request does not apply to you, then explain why it is not relevant on the application. Bankruptcy, a history of litigation, or disciplinary action will not necessarily disqualify you from membership. However, If any details are determined to be omitted, misleading, inaccurate, evasive or false, the application will be denied and your application fee will not be refunded.
